Manually Send a Form

00:00: This guide will walk you through the steps to on manually send a form to the patient. 00:05: On the Curogram App Dashboard, navigate to the conversations tab. 00:10: Click the new message icon. 00:13: Click "Chat with a Patient" button. 00:16: Please search for the patient's name. Once you have located it, click on it. 00:21: Click on the message icon button. 00:24: Navigate to the "send form" button and then choose what kind of form you want to send to the patient. This form varies from provider to provider.

This guide will walk you through the steps to on manually send a form to the patient.

Go to app.curogram.com

1. Click "Conversations"

On the Curogram App Dashboard, navigate to the conversations tab.

Click 'Conversations'

2. Click here

Once there, Click the new message icon.

Click here

3. Click "Chat with a Patient" button.

Click "Chat with a Patient" button.

Click 'Chat with a Patient' button.

4. Click the patient's name.

Navigate to the search field area and enter the patient’s name. Once you've seen the patient name, click on it.

Click the patient's name.

5. Click the message icon button.

On the patient dashboard, Click on the message icon button.

Click the message icon button.

6. Navigate here

Navigate to the "send form" button and then choose what kind of form you want to send to the patient. Do note that forms varies from provider to provider.
